if you want to off heal you need both+plague of mending, dark renewal is an option, but sometimes inst heal from renimation make a big difference& pom heals whole pt and by lots.

as rubies for offheal u dont need - Virulence(most useless ruby atm), Blood tap r1 max 2 is enough, Shadowy reach imo r2 is enough (unless u r full heal u wont use it since is easier to heal most times), Master summoner for lurker is pointless since it oly reduces it's cast time by 15%, Blood aegis is a must have since it keeps your shields/armour up, Seal of death makes your pet hit like a truck .

Don't forget to close aura when u heal.
